The Services This Service Agreement Will Provide:
Plan Management Services
Our Responsibilities:
Help you manage your Plan Managed NDIS funding
Pay your NDIS bills and reimbursements
Chat to you about your funding
Provide Guidance on what you can use your NDIS funding for
Provide funding updates
Send regular statments
Listen to any questions or concerns your might have
Respect you at all times
Your Responsibilities:
Provide Here2Home Plan Management with a copy of my NDIS Plan
Share any questions or concerns you have with us
Be respectful to our team
Let us know if there are any changes to the consent of your profile
Let us know if there are any changes to your funding or plan
How we pay your service providers
Your providers send their bills to invoices@here2home.com.au
You have the ability to set up your Here2Home Account to allow you to approve these bills before we pay them
We review the invoice to ensure it fits within your funds and goals
If the invoice fits within your Funding, Goals and NDIS Guidlines we will request the funding from the NDIS
The NDIS will then release the funds to us
We pay the provider
Payment for our services
Here2Home Plan Management will charge a monthly management free from your Choice and Control Budget - $104.45
Please note,
Here2Home Plan Management charges in line with the NDIS Price Guide. If the NDIS price guide changes our rates will change in line with this.
Plan Management Funding is included in your NDIS plan and there are no out of pocket expenses of Participants that access Plan Management services

It is important to us you are able to share you experience with us.
Here2Home may conduct check in calls to ask if we are providing you with a service fit for you.
You can also contact the NDIS to share any feedback or concerns you may have with our service.

Please note, it is your responsibility to let us know if there are any changes to consent.
Cancellation of Services
Both you and Here2Home Plan Management have the right to cancel service at any time.
30 days notice must be provided to cancel service by either party.
Cancellation of service can only be requested by a Main Profile Contact
By providing your signature below, you, as the participant (or participant representative), are agreeing to the information that has been present above.
Please upload a copy of the Partipants plan or email it to pm@here2home.com.au
