Sian Williams
Founder & CEO
My name is Sian and I founded Here2Home in 2019 after working in various positions since 2012 for State Government at Disability SA. I hold a Bachelor of Behavioural Studies (Psychology) and have a strong interest in how the mind works.

My 'why' for starting Here2Home began with a passion for quality over quantity, and striving to create an environment where Support Coordinators would continue to love the job that they started, and avoid unnecessary burn out.
​
I am a single mum of 3 children, so I understand the importance of creating a life where work and play is integrated. I am incredibly proud and deeply grateful for all the H2H members and community of people walking this path with me.​​​​​​​​

I am Erica, and I have 14 years of dedicated experience in the disability sector. I have experience in leadership and am passionate about driving positive change and fostering inclusive environments.
I firmly believe in leading by example, embodying the values I instil in the team. My leadership style centres on collaboration, transparency, and resilience, thriving on challenges as opportunities for growth and innovation.

Casey Robertson
Finance Officer
My name is Casey, and I am often described as a friendly and approachable person, who loves the outdoors and animals.

I have over 5 years of experience in the disability space and have completed a Bachelor of Business Management, which provides me with a well-rounded understanding of the business and finance environment.

Specific skills that I bring to the role are a strong set of analytical skills and high-level problem-solving skills. I consider myself to be tech savvy and have a strong ability to effectively communicate, and work as a team player.

Joseph Naa
Team Manager
My name is Joseph and I joined Here2Home in 2021, first as a Specialist Support Coordinator and a Support Coordinator. In 2022, I moved into the role of Team Manager.

I have earned my Bachelor of Behavioural Science and have commenced my postgraduate studies in Psychology. I have worked in the Human Services Sector in Queensland for 13 years, in Victoria for 6 years, and more than 2 years in South Australia.
I enjoy the company of people, riding my bike, reading books, and especially building great memories with my family through spending quality time, being present, and having lots of fun.

Terri Steer
Team Manager
I’m Terri and I have the privilege of supporting a wonderful team at H2H and am one of the Team Managers.
I am a developmental educator with over 25 years of experience in the disability sector. I studied Developmental Education at Flinders University and Rehabilitation at the University of Calgary, building on my previous career as a qualified early childhood educator.
Throughout my journey, I’ve worn many hats including – support coordinator, case manager, parenting practitioner, school leavers and employment supports facilitator, early childhood practitioner, and more recently service area manager.
I’ve had the privilege of working closely with families and participants across metro and regional areas in SA and VIC, particularly in the not-for-profit and health sectors.
With a deep knowledge in all things NDIS, I’m passionate about helping and empowering others to follow their dreams and achieve their goals.
When I’m not working, you’ll find me enjoying time with my family, pets, and friends, watching, and playing sports, or soaking up the beauty of the great outdoors.​​​​​​​​​​​​​

Elle Colmer
Compliance Officer
Elle commenced her career in the South Australian disability sector in 2015 after graduating from the university with a Bachelor of Psychological Science. The changing space of disability services at the time saw her working in various government departments, developing an extensive knowledge of the industry.
After taking a year off to travel overseas, Elle joined the Here2Home team upon her return in 2020 as a Specialist Support Coordinator. This allowed her to work more closely with participants and their families and assist them to feel supported and empowered to achieve their goals.
As the Here2Home team grew, Elle transitioned to the role of Compliance Officer, assisting to ensure we continue to provide quality work of a high standard. Elle enjoys being a part of a strong, supportive and cohesive team.

Carmelina Rosa
Administration Officer
My name is Carmelina, and I am the Administration Officer at Here2Home. I’ve had the privilege of serving in various roles at the Department of Communities and Social Inclusion – Disability Services. Commencing my employment on August 1, 1982. I transitioned through roles ranging from cleaner to pantry hand, and then moving into administrative positions. My employment within the government ended on June 30, 2019, when I gained employment in the private disability sector, under the NDIA.

Throughout my career, I have found fulfillment in dynamic learning environments and through collaboration. The autonomy and trust given to me by my Team at Here2home has been particularly rewarding. In my job, I get to see the direct results of my efforts, which is incredibly satisfying. I really enjoy the people I work with. Everyone works well independently but are also great when it comes to teamwork.

I have over 15 years’ experience in various positions in the Disability sector including but not limited to administration, intake, training, liaison, finance, and leadership roles.

Prior to working for Here2Home I worked in the retail space first as an assistant and then as 2IC, I am also a professionally qualified make-up artist and pride myself in building successful ongoing relationships with clients and colleagues.
The skills I believe I bring to my role are strong communication, attention to detail, conflict resolution/problem-solving skills and time management. I enjoy speaking with participants when taking the phones and, supporting the Here2Home team.

Lisa
Specialist Support Coordinator
In my role as a Specialist Support Coordinator with Here2Home, I have been privileged to enter the world of NDIS participants and be part of their unique journey in pursuing independence. It brings joy to all parties involved in their network of supports, seeing participants pursue and reach their NDIS goals.
With my diverse professional qualifications, including a registered Diploma of Counselling, as well as Diplomas in Management and Human Resources Management, I bring a well-rounded skill set to my role. I have also undergone advanced training in suicide prevention, mental health first aid, managing aggression and potential aggression (MAPA), safeguarding children, and Domestic Violence Response training facilitated by Lifeline.
Throughout my career, I have gained valuable expertise in supporting NDIS participants with Acquired Brain Injury, early onset Dementia, Multiple Sclerosis (MS), and Psychosocial Disability. By embracing the principle that every participant's needs are distinct, I am dedicated to providing compassionate, person-centered support that truly makes a positive difference in their lives by enabling them to exercise better choice and control.
Working at Here2Home is fulfilling for several reasons. Specialist Support Coordinator
A highlight for me being a NDIS Specialist Support Coordinator (SSC), is connecting and getting to know people from all walks of life. I learn from every person that I meet - the participant, their family members, communities, and all informal/formal supports e.g., therapists. In my practice I've seen how people change and progress in life, and I love to celebrate their successes with them.
I believe that it is important to listen and hear what you want from your NDIS supports, combining this with my knowledge and experience in the NDIS sector. By working together, I can connect you with the most appropriate services available in the community, to ensure you get the best support.
As an SSC, my practice is to deliver an expert approach to address participant’s complex support needs, while also identifying risks/potential risks in their environment. I support people with a range of physical, intellectual, and psychosocial disabilities, assisting to implement supports that aid in removing complexities they currently experience.
My experience extends to sourcing appropriate accommodation and engaging providers (Government, Private and/or Community Housing) and may also include Supported Independent Living (SIL) or Specialist Disability Accommodation (SDA) if required. Most importantly in my SSC role, I enjoy supporting participants to implement their NDIS plan to its full potential.
Outside of work I enjoy spending time doing activities with my children that they enjoy, I love to watch soccer and, spend as much time as I can gardening and cooking.

I am currently completing my final honour’s year in psychology, ideally my end goal is to practice as an organisational psychologist.
Currently I am working as a qualified Specialist Support Coordinator with Here2Home. Previous roles have included support work, mentoring and counselling. I have first-hand experience working in the mental health space, supporting a broad range of disabilities and this is an area I am passionate about.
I have a unique skillset and I am multilingual and language proficient in Dari, Farsi, Hindi, Urdu, and English. Through my tertiary education and lived experiences, my ability to delivery culturally competent supports is second to none.
I utilize an evidence-based person-centred approach in supporting my participants.

Specialist Support Coordinator
As a Specialist Support Coordinator with Here2Home, I love being part of a team where I am valued and supported to continually develop my knowledge and skills. The support that I am offered means that I can give an excellent service to all those that I support.
I enjoy working together with participants and their families by using the strength of active listening, and a curiosity that helps bring to light a participant's strengths and the environments that allow those strengths to flourish. My goal is to enable engagement in activities that bring joy and enhance wellbeing in a participant's everyday life.
My previous roles as a Disability and Youth Support Worker, Mental Health Triage Worker and Mental Health Clinician have helped me develop a solid foundation of knowledge about the Disability and Mental Health Sector.
I love being outdoors, bushwalking, growing my own food and natural health. I am also a qualified Homoeopath and Yoga and Meditation Teacher and Teacher Trainer. I play guitar and enjoy dancing and spending time with my cat George.

For four years from 2017 to 2020 I spent my time studying at UniSA, working toward, and completing a Bachelor of Social Work. Throughout these years of study, I gained a sound knowledge and understanding of person-centred care.
I have only worked in the Disability field and this work brings me great rewards, challenge, and a sense of achievement. Knowing I have helped someone else improve their quality of life and achieve their goals, gives me both personal and professional satisfaction. The knowledge I learnt yesterday, I continue to apply to my work today.
The diversity of working as a Specialist Support Coordinator allows me to learn about different cultures, lifestyles and increase my knowledge on how to achieve best practice and person-centred care. Every participant has a different situation and expected outcomes from their NDIS plan, I thoroughly enjoy learning about all different walks of life.

Alan
Specialist Support Coordinator
My journey to becoming a Specialist Support Coordinator (SSC), started with earning my Bachelor of Psychology (HONS), and master’s degree of Social Work at Flinders University and am officially registered for both degrees. I am a registered NDIS Positive Behaviour Support Practitioner and, have worked as a youth mentor and support worker in previous roles.
I practice from a holistic, environment and culture-focused, empowerment and human rights-based framework. I believe that people are the experts in their own lives and thus I don’t try to impose my own values or beliefs on other people. I will be guided by the participant and their family, and their goals.
As an SSC my experience in the NDIS space is varied and vast, while I specialise working with youth, as an SSC I provide support to participants of all ages and from different cultural backgrounds. Some of the areas I have experience with are, supporting participants living with complex mental health/psychosocial disorders, Korsakoff’s Syndrome, Autism, Intellectual Disability, and physical impairments.
I have experience supporting NDIS participants to find accommodation, that allows them to live more independently (SIL and SDA under NDIS). Often this includes supporting their transition from a government/health funded setting/structure. My SSC role can include engaging and working with the other government departments such as: the Office of the Public Advocate, SACAT, SA Health, Housing SA, Public Trustee, and the Department for Child Protection.
In my own time, I enjoy learning about the world, philosophy, different cultures and the role of education, learning, knowledge, and metaphysics in society. I love to have fun by engaging in outdoor activities such as sport, camping, surfing, swimming, and hiking. Indoors I like to play video games, watch movies, and spend time with family.

Specialist Support Coordinator
I have been working as a Specialist Support Coordinator / Support Coordinator for over two years, prior to this I was a support worker and administration officer. I sometimes feel that I “fell” into a role within Support Coordination, now I wouldn’t have it any other way.
Having already completed a Certificate IV in Disability, I am currently in the process of studying a Bachelor of Social Work through Flinders University. In the past I studied for two years toward becoming a registered nurse but decided, over time, that vocation was not the right path for me.
I have a passion for supporting young people and their families, building rapport, and navigating complex situations/dynamics. My aim is to ensure you meet your NDIS goals while we build both your formal and informal support networks.

Support Coordinator
I completed a bachelor’s degree in integrative counselling and psychotherapy in 2021, this serendipitously led me to my current role as a Support Coordinator which I love.
My experience has seen me work with children and adults, with varying needs and support requirements. My background has provided a wonderful underpinning to the work I do, as a naturally empathetic and compassionate person I find Support Coordination work meaningful and a role in which I am continuously learning and growing within. I enjoy seeing consistent, positive changes in the lives of individuals and families I work for/alongside.
My toddler keeps me busy when I'm not working, and so does my big dog Artie. We love spending time in the sunshine, at the beach, or among the green of the scrub. I have a love of cooking and sharing food (I'm a feeder, I can't help it!) and I also love art, I would love to study art therapy in the future but for now - messy play with my son will do. All of this keeps my cup full.

Support Coordinator
My journey into Support Coordination started with a Bachelor of Education Degree that led me through the Education system as a Primary School teacher. My specialty here was working with kids facing barriers that most kids did not need to worry about.
I moved from Education into healing therapies. I am a qualified Kinesiologist as well as Reiki Master. This has really deepened my insight into the effects barriers have on people.
I have more recently been a Youth Navigator and Complex Trauma practitioner working with young Indigenous adults.
I practice from a human rights framework that promotes the rights of people with disabilities to build a more accessible, more inclusive community. I believe they are the experts, and I will be guided by them, their family, and their NDIS goals.
As an NDIS Support Coordinator, I have experience in intellectual, cognitive, neurological, sensory, or physical impairments and psychiatric (psychosocial) conditions. I provide service to participants of all ages, from different cultural backgrounds and provide support to engage with services of their choice, to find appropriate accommodation (SIL, SDA,), engage with multiple sectors (Health, Legal, Employment, Education), and build their capacity to understand and navigate the NDIS system.
In my own time, I do what I can to take care of myself and enjoy life as much as possible. I paint, read, and write. I love the beach and if there is a Botanic Garden anywhere close, I am there. As I have built capacity for myself, I am daring to have fun and love hanging out with my kids and their partners.

Support Coordinator
My journey onto the NDIS landscape commenced five years ago where I provided holistic support and mentoring in my local community. This led to Support Coordination as I saw the need for people to be supported to navigate complex systems and processes so that they may access the support they are entitled to and live a better life. Here I have used my skills and knowledge to enable people to connect with their local community and services to meet their needs and expand their opportunities.
My professional and volunteer roles have given me the opportunity to work with people from a diverse range of backgrounds and abilities including, intellectual disability, sensory impairments, autism spectrum disorder, mobility impairments, and mental illness. However, I see people as so much more than these characteristics and view people as unique individuals with their own strengths, capacities and interests. I am committed to meeting people where they are at and working with them collaboratively to meet their goals.
I have personal experience in caring for a loved one with a disability, including the role of being a guardian and NDIS nominee. This has given me skills in working with family and informal supports in tandem with NDIS participants to build their skills and become more independent.
I live in the Southern region of Adelaide where I enjoy the beaches and hiking in the scrub. Other interests include creative pursuits such as mosaics, painting and singing in a local cover band. I am committed to furthering my education and study Social Science and Counselling at University. I am also an active volunteer and have a strong purpose to be of service to others.

A big hello to all! My journey to Support Coordination really began when I was in school. Having disabilities in a school environment, I often felt misunderstood and was labelled as a naughty kid when in fact I felt trapped by my limitations, acting out on that emotion. That was until one person saw past all that offering understanding and compassion opening my world up by looking past limitations. I knew from that point I wanted to assist others who also may feel like something can’t be achieved.
One of the big things I bring to my role as a support coordinator is understanding from having that lived experience, and having children with disabilities. I specialise in mental health and psychosocial disability but by no means am I limited to just this area!I have worked alongside a variety of people and will support anyone regardless of age, gender, culture, background, sexuality or disability to achieve their goals. I am very open and honestand call things how I see them.

My name is Amelia, I have worked in Home Services & Support Coordination since 2020. Prior to that I wasa Support Worker/Team Leader in the disability community.
My strengths include communication, organisation, and time management. I am an approachable and friendly person, which I think is a strength when it comes to building & maintaining positive relationships with participants and their support networks.
I approach work relationships by being open and ready to take the time with participants, getting to know them, listening to what it is they need support with and building trust. No one person is the same and so I believe it is important that I follow the participants lead when it comes to supporting them.
I am passionate about working within the whole disability field, but hold more experience in supporting and assisting those who have ABI’s, Rett Syndrome, CP, Dementia and MS. I have also supported clients that are hearing impaired and use sign language, clients that are on the spectrum with ASD and those with Mental Health conditions and complex behaviours.
At my core is a passion for person centred care and I always give 110% and strive to support others as best as I can.

My name is Belinda, and I am a Support Coordinator at Here2Home. My background is in Early Childhood education, however I transitioned into Support Coordination as I am driven by the belief that I can positively impact people's lives in a supportive capacity.

My name is Dani and I work as a Support Coordinator within the Here2Home team. My background is in clinical research where I’ve spent the past 5 years working in pharmaceutical and device clinical trials within the specialties of Respiratory andCardiology.

Support Coordinator
My name is Nele, my work in the disability space started in 2018 when my husband became an NDIS client due to his EO Alzheimer’s diagnosis. As his main carer, support coordinator, spokesperson and advocate our ever-changing journey began.
I gained a sound knowledge and understanding of complex person-centred care within the disability realm. I have experience with a range of disabilities and/or health conditions, as well as Carers Support. My roles within the disability sector include: advocate, support coordinator, support worker, personal carer, navigator of systems and services, financial manager, adviser on home modifications and AT.
My professional history is anchored in design, creative thinking and the arts, with my professional practice transferring into overseas community development program work with an international NGO for 16 years, which lead to a sound knowledge of capacity building, empowerment, egalitarianism, developing networks and strong relationships, human rights, cultural protocol, humanitarian relief.
Experience has taught me that every person has a different situation and expected outcomes from their support programs.I am a patient listener, respectful communicator, and collaborator, lateral thinker and researcher. I strive to find the best solutions and advocate strongly for them. I value resilience, transparency, honesty, respect, inclusiveness.Life is about choice, equality, empowerment and community.

Support Coordinator
My name is Patricia, and I am originally from Brazil. I graduated as a Clinical Psychologist in1999 and had my own practice from 1995 - 2003.
In Brazil I worked with children experiencing PTSD, until I migrated to Australia in 2003. I lived in Sydney for 5 years where I studied English and Human Resources and Management; and later moved to Adelaide in 2008.
I started working in the disability sector in 2019 and instantly fell in love with the job. I have been supporting people living with Psychosocial disabilities and Intellectual disabilities, as well as people with very complex behaviours. Primarily my role revolves around working with the individual and the family unit to explore and link with mainstream and community services, building capacity to exercise choice and control, and helping to resolve and overcome points of conflict and crisis situations.
I also work with individuals and their families to build their capacity and understanding of the NDIS, in addition to conducting direct service delivery, and conducting assessments and reports.

Support Coordinator
My name is Rachael and I am a Support Coordinator at Here2Home. My journey into support coordination started when I became involved in the care of a family member with multiple complex disabilities. Through this process, I gained a new appreciation of how fortunate we are in Australia to have access to essential supports when we need them. I also realised that up-to-date information and accurate advice is invaluable in ensuring the optimal implementation of anNDIS plan, and became passionate about developing my own understanding of how NDIS participants can be supported to utilise their funding effectively and in ways that work for them.
Prior to my transition into support coordination, I was a primary and middle school teacher for over eleven years and have firsthand experience around how assistive technology and suitable support staff can improve social, emotional and educational outcomes for students with physical disabilities and neuro-developmental disorders. I have also provided one-on-one support to children with complex disabilities as a carer, and have been employed in crisis care and residential care settings as a support worker.
As a Support Coordinator, my passion is supportingparticipants of all ages and their families to feel confident in their ability to make informed andwell-educated decisions around every aspect of their NDIS plan

As an NDIS Support Coordinator, I draw on my background in property management to effectively connect with clients, families, and service providers. This experience has equipped me with strong communication and organisational skills, essential for coordinating support services. I approach problem-solving and conflict resolution with a focus on understanding and advocating for participants’ needs.
Recently, I completed The Growing Spaces supportcoordination course, which deepened my knowledge of the NDIS framework and best practices. My passion for this role is further shaped by my lived experience as a parent to a beautiful non-speaking, neurodiverse child. This journey has enhanced my understanding of the challenges families face and fuels my commitment to supporting individuals with disabilities.
I believe that empathy and understanding are at the heart of effective support, and I strive to create a safe and welcoming environment where clients feel heard and valued. My goal is to empower individuals, families, carers, and guardians by helping them access the resources and services they need to thrive.​​​​

My name is Leah, I have a passion for mental health and wellbeing and understanding how the brain works, which lead me to study psychology in high school, then in university.
I have experience as a support worker in a youth residential facility in Queensland, then over 3 years experience as an NDIS support coordinator in Melbourne, where I currently reside.
I love this role because I love working with clients long-term and developing a good, stable, consistent working relationship that results in people achieving their goals. I also enjoy doing all those admin tasks that others may find boring or overwhelming!
In my spare time I am usually hanging out with my 3-year-old son and hubby, I also try to go to a movie or dinner out with friends when I can. My perfect day would be crafting and bingeing a tv show at home on a cozy rainy day.

With a strong foundation in social work and education, I am passionate about empowering individualsand fostering positive change. My diverse experience has provided me with a comprehensive understanding of various needs and strategies.
In educational settings, I have supported students through mental health challenges, promoting their academic and personal growth. My teaching experience deepened my understanding of learning development and diverse learning needs and techniques, enhancing my ability to support participants effectively.
In out-of-home care, I worked closely with foster families and support services to ensure the safety and well-being of individuals.

My name is Kylie and in my current role with Here2Home I work as a NDIS Specialist SupportCoordinator, I enjoy this work as it provides opportunity for me to support people to live their best life, their way.

My name is Lauren, I have 3 years’ experience as a support coordinator, with additional experience in the disability sector in various roles such as Business Development and a Team Leader.

Support Coordinator
Hi, I'm Janet. Over the past 20 years, my career has been dedicated to disability, health, community,and the justice sector. I have advocated and supported individuals, fostering environments where they can learn and build within them the confidence to have choice and control and feel valued.
My inspiration stemmed from watching my mother’s impactful work in a nursing home. Her ability to make residents feel seen and cherished instilled in me an understanding that true care goes beyond physical assistance—it means genuine human connection.
This shaped my passion for uplifting those whose voices often go unheard. I have also faced multiple struggles firsthand that have deepened my empathy and allowed me to help others who have shared similar experiences. I am now pursuing a degree in psychology, my aspiration is towards neuropsychology and behaviour modification.
I have extensive knowledge in the NDIS space, I have worked as a Local AreaCoordinator; a Coordinator dealing with the welfare of those that have been incarcerated; worked inCoaching; and as a Nurse dealing with everyday health concerns of the community in all its complexities.My passion is to continue my mother's legacy, ensuring every individual I support knows their worth, strength and can achieve their goals. Support Coordinator
I have worked with Here2Home for well over a year, providing support coordination to various NDIS participants. Before this, I previously worked as a support worker, so I understand how care worker agencies operate and had a lot of experience with the NDIA during this time.
My specialty is anything mental health related, especially ASD and ADHD. I believe in treating people kindly and with respect, no matter their background. I always aim to come from an understanding place and if I don’t understand, I am always willing to listen, learn and fill gaps in my knowledge.
I have lived experience with ADHD, and I know how difficult navigating complex systems can be, especially when mental health is impacted. Due to this, I endeavour to always make it as easy as possible for people to be able to navigate their NDIS plans and connect to the supports that they need.
I also help my partner look after their children, who also have their own NDIS plans which has given me additional experience with the NDIA in a different setting.

My name is Nadia and I have worked in the disability and mental health sector for about 6 years, prior to that I was in Youth Justice for 3 years.
My passion in the disability sector is with mental health. I have first hand lived experiencewith people close to me who have differing mental health conditions.
I enjoy my role as asupport coordinator because I like being able to support people and families who arestruggling, and help to make a meaningful difference. I am also a mum and animal lover!
MIKE
NDIS Report Writer
My name is Mike, and I’m an NDIS Report Writer at Here2Home. I have over 30 years experience in the disability space, both here and he UK. I completed my Bsc (Hons) Psychology in the 1990s, during which I discovered my passion for working in disability.
Over this time I have worked in the private sector and mostly SA state and the NDIS. I began in support roles and moved around into case management and administrative roles; most recently as a NDIA senior planner. I bring a strong set of analytical and problem solving skills, report writing skills and a firm grasp of the NDIS Act and the NDIS Rules.
