SUPPORT COORDINATION
Support coordination may involve connecting you to service providers and support, and assisting you to make the most of your NDIS plan budget.
Here2Home also provide a higher level of support coordination for people with circumstances that are more complex. This is delivered by a Specialist Support Coordinator with a high level of experience who can support you to address barriers and resolve points of crisis.
SOCIAL WORK
Many of our specialist support coordinators are also qualified social workers. Social workers are able to support you with things that sit outside the scope of a support coordinator. This may include things such as advocacy, assessments, community resources, therapeutic support and counselling.
